What do I need to be evaluated by a physical therapist?

Typically, all a person needs is a prescription from a physician, physician’s assistant, or nurse practitioner.

 

What insurances do you accept?

Southern Delaware Physical Therapy accepts most insurances including Medicare, Medicaid, Diamond State Partners, Blue Cross/Blue Shield, Tri-Care, Coventry, Aetna, Optimum Choice/MAMSI, Alliance, United Healthcare, Delaware Physicians Care Inc. (DPCI), Cigna, and others.  We also participate with claims after Motor Vehicle Accidents and Work-Related Injuries.

What should I bring to my appointment?

 Please bring your prescription for physical therapy and your insurance cards.

 

What should I wear to my appointment?

 Please wear comfortable clothing that will allow the physical therapist to fully evaluate the specific body region for which you will be treated.

 

How long will my appointment last?

 The first appointment will be an evaluation that will take approximately 45 to 75 minutes.  Subsequent appointments, which you will schedule after the evaluation, will last 30 to 45 minutes.

 

How will my doctor know how I’m doing?

Your referring physician will receive a copy of your evaluation and we will keep in contact with him/her during the course of treatment.  If you have a follow-up visit with your doctor during therapy, we will provide him/her with a comprehensive Progress Note detailing your improvement.
